For all entities/individuals who do business with CPCHAP, including our current employees, we ask you complete item A and B below.

A. Please read our Corporate Compliance Program Statement and sign the second page and return the first and second page back to us at info@cpchap.org

B. Complete the web-based Fraud Waste and Abuse (FWA) training course offered by the CMS MLN (Medicare Learning Network) at https://learner.mlnlms.com/Default.aspx

Upon passing the test given at the end of the training course, the MLN system will generate a certificate of completion.  Please save a copy of the certificate of completion and send it to info@cpchap.org

The Compliance Program Elements: I to VIII corrected insufficiencies identified by the Office of Medicaid Inspector General (OMIG) during the 2017 Compliance Program Review and Assessment.
